Financial & Insurance

Fees for services rendered are to be paid at the time of service.

Please remember that health insurance contracts are made between the insurance company and the patient, and it is your responsibility to see that claims are filed. If you provide us with the needed information we will be happy to file claims for any office or hospital services we provide. With “traditional insurance” you can file claims for office visits by attaching the encounter form summary to your standard insurance form.

There are a variety of health care plans (PPO’s, HMO’s, etc.) now available. A list of the plans we participate in is listed below. Some of these plans require that we submit all claims directly, and of course, with these plans we will do so. Many plans require you to make a co-payment at each visit. Co-payments are to be paid at the time of the visit.

It is now more important than ever for you to fully understand your insurance agreement. Failure to comply with the contract requirements can result in penalties to you in the form of reduced or disallowed benefits.
